What type of passport can I get? 

Passport Acceptance Service processes only new applications, long-time (more than 15 years) expired applications and youth applications.

What are the requirements for passports?

1. Complete all necessary forms.

Forms must be printed single sided and should be completed in black ink.

• Application for New Passport and Child Renewals (Form DS-11)
• Statement of Consent for a Child Passport (DS-3053)
• Additional forms (U.S. Dept. of State website)

2. Provide proof of citizenship (original and a copy). 

Forms Certified birth certificate, passport, or naturalization certificate. See Get Citizenship Evidence for a U.S. Passport for more detailed information. 

• For minors 15 and under, child and both parents must be present.
• For minors 16-17, child and one parent must be present.

3. Provide proof of identity (original and a copy). 

Valid government issued picture ID.

• For minors, parents must present valid ID.

4. A Passport photo.

• Passport photos available on-site for $5.00

5. Pay the U.S. Department of State by check or money order.

The U.S. Department of State passport fees must be paid by check or money order only. Use the Passport Fees Calculator to estimate your cost.

6. Pay the Beaver Creek Public Library.

• Facility Fee: $35.00 Must be paid by cash or check.
• Photocopying Fee (if necessary): $0.15 per page
• Passport photos (if necessary): $5.00 for 2 photos.
